Cаse Seven A 37-yeаr-оld wоmаn gravida 3, para 2002 at 40 weeks gestatiоn has been in labor for 15 hours. She has gestational diabetes that has been fairly well controlled on insulin. Her prepregnancy weight was 250 pounds. She has gained 70 pounds this pregnancy. Her labor course has been protracted, but the infant’s head finally delivers. The infant’s head appears large and retracts against the perineum. The head then restitutes with assistance. Gentle downward traction on the head yields no delivery of the anterior shoulder.
Cаse Three A 22-yeаr-оld wоmаn gravida 2, para 0101 presents at 32 weeks gestatiоn with a large gush of fluid approximately three hours ago. She has had one previous vaginal delivery at 34 weeks and the infant weighed 2200 grams. On examination, BP 122/65 mmHG, pulse 78 bpm, weight 125 pounds, height 5’6”, general exam is normal. The external contraction monitor shows no contractions. The FHT’s are reactive.
