Rаymоnd is а 67-yeаr-оld retired pastоr who was referred by his ENT after reporting a 7-month history of progressive voice changes. He describes his voice as “weak, breathy, and effortful to project.” He lives alone and reports that he has stopped attending his weekly Bible study group because he is embarrassed by his voice. Flexible laryngoscopy revealed right vocal fold paralysis with a significant glottic gap at midline. Aerodynamic analysis shows severely reduced MPT (6 seconds), elevated s/z ratio (2.1), and CAPE-V overall severity of 74/100 with dominant breathiness (82/100) and mild strain (22/100). Voice stimulability testing with a pushing/pulling technique and lateral digital manipulation produced modest but audible improvement in voice quality. What is your preliminary diagnosis?
