Operаtive Repоrt DIAGNOSIS: Benign prоstаtic hypertrоphy PROCEDURE: Robotic lаparoscopic simple prostatectomy INDICATIONS: This 75-year-old male had a history of benign prostatic hyperplasia and urinary retention. He had a 109-gr prostate. He was here now for a simple prostatectomy. FINDINGS: The patient underwent a simple prostatectomy without difficulty. DESCRIPTION OF PROCEDURE: The patient was brought to the operating suite and placed in supine position. He was given general anesthesia and endotracheally intubated. He was prepped and draped in the usual fashion. A Foley catheter was placed in his bladder through the penile urethra. A small incision was made in the midline just above the umbilicus using the Hasson technique. A 12-mm port was placed. His abdomen was insufflated. A 0-degree laparoscope was inserted into the abdomen, and the abdomen was inspected. Next, a 7-mm port was placed on the left side at the anterior iliac crest line, and a 12-mm port was placed at the midclavicular line. On the right side, 7-mm ports were placed at the midclavicular line and at the level of the anterior iliac crest. The 0-degree laparoscope was inserted in the midline port, and the robot was docked. The patient was put in severe Trendelenburg, and the scissors and Maryland graspers were attached. The bladder was filled with saline. A horizontal incision above the bladder neck was created, and the bladder was entered. The prostate with the adenoma was seen. The Foley was removed. A 0 prolene suture was stitched into the adenoma. Using the suture to manipulate the adenoma, the mucosa around the adenoma was incised circumferentially around the bladder neck. The adenoma was then dissected away from the capsule. The adenoma was put into the endopouch. Small blood vessels were fulgurated. The adenoma was completely removed. Some of the bleeding vessels were then suture-ligated. The mucosa of the bladder neck was attached to the capsule of the prostate. The bladder was closed with 2-0 Vicryl in two layers. A 20-French Foley was used as a suprapubic urinary catheter. It was inserted 2 cm above the pubic symphysis and into the dome of the bladder. A pursestring suture was placed around the catheter. A 19-French drain was placed. The robot was then undocked. The ports were removed. The prostate was removed in the midline port. The fascia was closed using 2-0 suture in a subcuticular fashion in the 12-mm port. The skin was closed using 4-0 suture in subcuticular fashion. We then placed tissue adhesive over the incision. The needle and sponge counts were correct. The patient tolerated the procedure well.
