Presenting Concerns: Today Bill has ongoing complaints of persistent shortness of breath and chronic cough. He was diagnosed with COPD five years ago and started on Spiriva Respimat. Bill was also prescribed an albuterol MDI for use as needed to help relieve symptoms. Current Medications: Spiriva Respimat 2.5 mcg/actuations- 2 inhalations daily Albuterol 90 mcg/actuation- 2 inhalations every 4 hours as needed Amlodipine 5 mg tab daily for blood pressure Atorvastatin 80 mg tab daily for high cholesterol   Social History: Bill works as a carpenter and has been woodworking for the past 35 years.Tobacco use: 1 pack of cigarettes per day x 40 years.No alcohol use Medication adherence: Patient is adherent to Spiriva and is using correct inhaler technique.
