Use the scenаriо belоw tо аnswer аll parts of the question. A father (35yr, male) and his young son (1yr, male) are admitted to the emergency room. The father presents with a persistent respiratory infection characterized by bronchitis and his chest x-ray shows early occlusions indicative of pneumonia. The child is severely dehydrated with debilitating diarrhea that has shown some blood in the child’s stool.  A) You ask the patient about his recent history and he states that he and his son had just returned from visiting family in Thailand, where his parents run a sugar cane and dairy farm. He was out admiring the farm when a rain and wind storm arrived. What microbe would you suspect of causing his respiratory infection? What is your evidence for choosing this microbe? B) You inquire about his son’s condition and ask if they have ingested anything unusual in the last week. He recites a list of dishes that his family prepared for them. After additional questioning he does admit that on the farm his family commonly drinks raw (un-pasteurized) milk and that both he and his son had drank the milk. The father admits that he also had a bad case of diarrhea but he drank some Pepto Bismol (bismuth salicylate) and the diarrhea went away. What do you think is the cause of the son’s current condition? What is your evidence? C) The doctor (a 1st year intern) orders the child to be placed on a strong course of amoxicillin and tetracycline in addition to giving the child IV fluids. Within a couple of hours, the diarrhea has become significantly worse and now shows much more blood in the stool. Additionally, the child is now showing signs of systematic inflammation. Stool samples reveal the presence of Shiga toxin. Do you change your assumption of what microbe is causing this disease? If so, what microbe do you think is responsible now? If not, what is your evidence for your reasoning? What could be causing the now fast-spreading systemic inflammation that the patient is experiencing?
