Additiоnаl Cаse Infоrmаtiоn Interventions completed so far in the ER: -20 gauge IV inserted to R arm -CBC, CMP, PT/INT/aPTT, Urinalysis -EKG -3 mg Morphine IV was given once for pain on pt’s arrival Diagnostic Imaging: AP/Lateral Left hip X-Ray: Interpretation: “This AP L hip x-ray reveals an oblique, comminuted, displaced subtrochanteric femur fracture. A single fracture fragment appears significantly displaced/angulated between the distal and proximal fracture segments. The joint space appears preserved and aligned, without evidence of intra-articular fracture.” The following additional radiographs were completed: -3-view L femur x-ray - Results: see L hip x-ray results. Beyond previously noted fracture, there were no additional distal fractures or identified abnormalities to the femur. -2-view L tib/fib x-ray - Results: unremarkable, no acute injury, dislocation, or fracture -3-view L knee x-ray - Results: joint space narrowing consistent with moderate osteoarthritis seen; otherwise no acute injury, dislocation, or fracture noted. -AP Pelvis x-ray - Results: see L hip x-ray results. Beyond previously noted fracture, there were no further identifiable pelvic fractures. -CT C-spine w/o contrast - Results: notable degenerative disc disease and joint space narrowing throughout cervical spine, worst at C5/C6; otherwise no acute injury, dislocation, compression, or fracture noted -CT head w/o contrast - Results: Incidental finding of intracranial calcifications in multiple vessels suggestive of atherosclerosis. This should be correlated with patient’s medical history; otherwise no acute cardiopulmonary abnormality or fracture Laboratory Diagnostics: Test Results Reference CBC WBC 8.1 RBC 4.0 Hgb: 12 Hct 35.8% MCV 84 MCHC 29 Platelets 225 Neutrophils 56% Lymphocytes 32% Monocytes 8% Eosinophils 2% Basophils 1% 4.0-11 3.9-5.1 12-16 35-45% 80-100 27-34 150-450 40-60% 20-40% 2-8% 1-4% 0.5-1% CMP Sodium 136 mmol/L Potassium 3.9 mmol/L Chloride 98 mmol/L CO2 22 mmol/L BUN 20 mg/dL Creatinine 1.3 mg/dL Glucose 110 mg/dL Calcium 9.2 mg/dL Alk phos 108 U/L ALT 29 U/L AST 20 U/L Albumin 3.4 g/dL Total Protein 6.0 g/dL Total bilirubin 0.5 g/dL 135-145 mmol/L 3.5-5 mmol/L 96-106 mmol/L 20-30 mmol/L 6-20 mg/dL 0.6-1.3 mg/dL 60-120 mg/dL 8.5-10.2 mg/dL 30-120 U/L 4-36 U/L 8-33 U/L 3.4-5.4 g/dL 6.0-8.3 g/dL 0.1-1.2 mg/dL PT / INR PT: 12 INR: 1.1 PT: 11-13.5 seconds INR: 1-1.2 aPTT 29 seconds 22–39 seconds Urinalysis Color: yellow Appearance: clear Leukocyte esterase: 0 Nitrites (mg/dL): 0 Urobilinogen (mg/dL): 0.8 Protein (mg/dL): negative pH: 7 Blood (mg/dL): neg Specific gravity: 1.020 Ketones (mg/dL): neg Bilirubin (mg/dL): neg Glucose (mg/dL): neg RBC: 1 WBC: 1 Squamous epi cells: few Bacteria: none Mucus: none Clear to yellow Clear Negative Negative 0.2-1 Negative 5.0-9.0 Negative 1.005-1.030 Negative Negative Negative 0-5/HPF 0-3/HPF Negative Negative Negative EKG NSR rate or 72 bpm without ectopy, arrhythmia.
