Yоu аre а newly grаduated Pathоlоgists’ Assistant (PA) working the 2:00 pm–10:30 pm shift. On this particular evening, your lead PA (the only other PA schedule to work the evening shift), has called out sick. All staff Pathologists have left at their usual time of 5:00 pm. As the only PA on duty in the entire hospital, you are now acting as the lead PA, responsible for answering questions from the histotechnologists and lab assistants. At 9:30 pm, one hour before your shift ends, you have an uncompleted workload full of other patient's breast biopsies, and GI biopsies, the priority specimens for the day. At 9:35 pm, the lead Histotechnologist rushes in carrying an extremely complex en bloc head-and-neck resection specimen: a total glossectomy (tongue resection), bilateral neck dissection, and partial floor-of-mouth resection. She reports that the specimen has already been accessioned into the computer and needs to be completed right now as a STAT case. Moments later, you receive a phone call from the operating surgeon of that STAT complex head and neck case—who is widely known for being impatient, aggressive and abrupt in their communications with all staff. The surgeon angrily tells you that this case is “top priority” and must be grossed in tonight, including all margins, because the patient is scheduled for follow-up surgery tomorrow morning at 9 am to access the margin clearance of the tumor and to guide further tumor debulking. He promptly hangs up. The lead histotechnologist tells you they will run a late processor for the block processing from the complex specimen and can have the slides to a Pathologists by 8am, so just "Gross the specimen now, so we won't get in trouble with this Surgeon".  You now face the decision of whether or not to attempt a complicated, time-consuming, high-risk specimen that you may have never encountered before without a Pathologist or lead PA available on-site to supervise or confirm your work—while under explicit pressure from a notoriously difficult surgeon who demands diagnostic accuracy, all while knowing you also have to complete your normal priority work (breast and GI biopsies from other patients).   What would you do in this situation?  Ensure you address at least ONE main medical ethical principal (Autonomy/Beneficence/Justice/Non-Maleficence) that will be at stake in this situation?    Things to consider while writing your essay answer: -Your competency level as a Pathologists' Assistant -Who you might contact -The consequences of an inaccurate diagnosis  -Your work ethic and professional behavior -The other patients who are waiting for their cancer diagnosis from those yet to be grossed biopsies -The intuitional hierarchy and if you would lose your job for the wrong decision -And any other ethical concern you may have about this situation
